Is it possible to add this field to the PO form?

It is already there as an invisible field on the RTM. Make sure you have the latest version of the RTM and you should be able to see the field and make it visible in the forms editor.

Actually, I can't find the invisible field. The date of the BKPOB4.RTM in 06/2002. Is there a more recent form?

yes, BKPOB4.RTX will be the newer version. Rename to RTM and then edit.
