How do we get the fax and phone information to print on our sales orders and therefore corresponding packing slips and invoices when we enter them in the fields provided in new customer entry?

All those fields are already on the latest RTMs. To make sure you have the latest, copy the corresponding BKSO...RTX to the RTM with the same name. (In Evo, the RTMs are T6...rather than BK...)
