Has anyone had an issue with duplicate lines automatically placed on a Sales Order. This has arisen twice this week for 2 different customers whereas the Line 1 of the order was duplicated with another Line 1 of the same info effectively doubling the order.

Did you have many lines on the Order? I would make sure all the current patches are installed using SM-V then Reindex the BKARINVL file. Use maintain Database and make sure there isn't any garbage in any records (file is big im sure so just skim over it)Then monitor to see if it happens again.
