With the update package dated 6/7/04, both the SA-M and SA-N screens disappear after completing the form within Evolution.  Both screens run flawlessly in regular DBA.  A fix is appreciated!

Happening here, also.  We use these reports for our monthly records.  Has it been resolved yet?
