Here's a good one.
I have a finished good that is built using features and options.  One of the options is a B inventory item.  For each finished good there can be 2ea of the B item.
I enter a sales order for 2ea of the finished good, with a quanty of 4ea of the B items.  I created a work order from the sales order, which brings in the B item components.  When I close out the work order, only half of the B item components are issued (i.e.,  standoff:  required 8, issued 4).  
I am using EVO with the latest updates, but I believe the bug is also in classic DBA.  This has caused some inventory problems, of course.
