Problem backflushing materials

When "backflushing" materials to a work order, on occassion, an item will not post to the work order as being issued, however the items transactions will state that it was indeed issued.
 
The item was coded to be backflushed as required in the BOM.
 
Any help would be appreciated.

Does the transaction show in JC-E but not in the % Issued on the Materials button in WO-A or is it neither place?  Lot or Serial control?  Any other pattern for these as compared to parts that process properly?

The item issued does not appear in JC-E or as a % in WO-A.  The items do not have lot or serial control.   This typically has been taking place through "backflushing".  The latest example showed up in the transactions button as "0" quantity issued to the work order.  Our materials manager then issued the item through WO-G and it appeared as a transaction but still does not appear in JC-E or WO-A.  It seems as though something is blocking the rest of the process of posting to WO.   The materials manager has been finding these issues about once a week for the last three months.  I've been holding them looking for something in common however have not found anything.
 
Again, any help would be appreciated.
