Purchase to Work Order ( J Transaction) of Type N

When purchasing to a WO ( J  type transaction) of Type N (sometimes R) parts:
 
Costs in tables INVTXN & OUTPROC are being incorrectly populated with strange values (usually much higher) than the transaction actually occurred and is correctly stored in BKAPPOL.
 
This has been documented to happen at two different companies that our firm has worked with.  At one it only faulted with type N parts but the other has seen this on type R parts in the past as well.
 
Occurrences are infrequent and it's difficult to find a rhyme or reason.  Last occurrence was this last week and the system was already upgraded to the latest ISTECH updates.
 
Has anyone else seen this?
 
Lynn, I know it's hard to debug an error that's not automatically repeatable, but is there any research going on for this issue?
