We have notice this issue since the last update (SP1 July??)
 
A PO is created, saved and printed.
Weeks later when our staff tries to received the PO, it is not there. They open PO-A an the PO is not there.
Because the items are on the company and need to be receive they create another PO with the same number, and items.
 
When they are trying to receive it the items appear twice.

So it is somehow losing the header but not the lines, then when it is re-entered with the same number it finds the new lines plus the original lines.  Which explains the doubling of lines but not how it is getting dropped in the first place.  Is the PO being edited after the initial entry?

I don't know. They probably not remember either.
 
I have ask the Purchasing staff to do not create another PO with the same number, until I check the tables.
 
It does not happen often but it has become a concern.
