Cannot access Forms Editor in DBA Classic

I get Runtime Error 216 whenever I try to run the forms editor in DBA Classic.  I had to reinstall DBA a couple months ago and ever since I get this error message.  Maybe the forms editor program is missing or corrupted.  What is the program name and can I replace it?  Any other suggestions?

Classic calls ENDUSER for the forms editor.

It is actually embedded within the TP5WDBA.EXE.  There should be a backup coipy of the EXE on your CD.

I recently installed the complete version of the 9/25 update hoping that this may correct the problem.   It did not but, I now get a general protection fault error instead of the timeout error when ENDUSER tries to load in both Classic and Evo.    
 
You indicated that the forms editor was imbedded in Tp5wdba.exe so I replaced the Tp5wdba.exe in the DBAMFG folder with a copy from the orignial CD as you suggested but that didn't help.
 
I noticed that ENDUSER.EXE is also in the DBAMFG folder as a separate program.  Should it be in both places?
