WO-J issueing more materials per work order

This issue started after the last EVO update.  It happens sporadically. We sell a portion of an open work order, but EVO issues multiples of the BOM materials/labor.  This throws our COGS/WIP off.  
 
Has anyone else experienced this issue?

WO-J is Close Work order.  It does not issue any materials.
