STATEMENT PRINT OUT WITH CODE NOT CUSTOMER NAME

Our statements only print out with code and not the customer name.  How do I fix this?

In DBA you can't.  The Evo-ERP forms editor can add data fields to the forms but DBA can't.

We have always been able to print out statements for our sales orders, what in the world changed in DBA so we can't?

By my records you are still using the 2002 or 2004 version of DBA because you have never purchased our updates so nothing has changed in almost 20 years.

This just makes no sense since two weeks ago I could print out sales statements and now the statements just print out like this
 
BKAR_CUSTNAME
CUST_ADD(1)
CUST_ADD(2)
CUST_ADD(3)
CUST_ADD(4)
CNTRY
 
And the rest of the statement is like this. So no name, no balance.

i'd try restoring a copy of the RTM from an old backup just to rule that out
