Greetings,
 
I've noticed that sometimes in DBA it will prompt you for which form to use and sometimes it does not.  Totally depends on the type of form being used.  How would I determine which form is being used if it does not prompt me?
 
For example, SO-O-A and SO-O-E, which I use frequently, don't tell me which form is being used.  I looked in the Pervasive monitor for open files, but no forms were open.
 
Any help would be appreciated.

If the format you have chosen in the defaults is a graphical format with an RTM associated with it, the RTM name will be displayed and can be changed when printing.  If no form name is displayed, you are using a text format which is hard coded into the program and can't be changed by the end user forms editor.  You could go into the defaults and change to a graphical format and see if there is one that will suit your needs.
